It looks like you provided more answer options for this question than any other item in the entire survey, while it affects relatively few people. I'm concerned that this will cause quite a bit of mental fatigue. To my read, there could be a little redundancy with your suggested answer options, for example: maybe enjoyed/disliked overall is covered in some of the other reactions? Or enjoyed/disliked is less productive than the specific TDM functions? Would you like to try and cut down the answer options? I'd prefer 8-9. If you'd prefer, I can try to do it instead!

Can you clarify what 'more collaborative/player input plots' mean? I think the way it's phrased, and in the context of the longer survey, players might not be sure whether you mean Player Plots (e.g., my powers have gone wild, now everyone has turned into a talking cat) or Fractured World-type AUs where players can go hogwild. Possibly you mean something else not covered by either of those things?

Is there any particular reason why you want to say 'strongly prefer not' rather than 'not want?' I would like to address concerns about offensive material, OOC/player trauma triggers, etc. more explicitly.
[APP] SPLIT the questions into whether or not the surveyee has actually filled out the app. I'm not sure if there is a better way to implement this, but I think it could be helpful to see if there are correlations between preferences and whether someone has actually used the app. Ex: [...]
Do you think it'll be OK if I just add a "Have you filled out the new application yet?" question, with Yes/No/Yes but not yet submitted. That way I don't have to add 2-4 more similar-looking questions?
